Consultation
For your initial appointment we highly recommend a pre-treatment consultation. This visit will provide you the opportunity to become familiar with our office, the treating doctor and our staff. During this appointment the doctors will review your concerns and familiarize themselves with your medical history. Many times questions arise that require our doctors to contact referring doctors, primary care physicians or possibly seek further diagnostic data. We will be able to discuss, confirm and explain your diagnosis and the various treatment options that may be available to you. During this visit you may find it advantageous to review and clarify your insurance coverage with our business staff. We will be happy to assist you in optimizing your insurance benefits. On occasion surgery may be performed the same day as the consultation.
Please assist our business staff by providing the following information at the time of your consultation:
If you have medical and/or dental insurance, it is important that you bring the necessary completed forms. This will save time and allow us to promptly process your health claims.
Your current insurance card with the appropriate insurance referral if applicable
Your surgical referral slip and any X-rays if applicable
A list of all medications you are presently taking
IMPORTANT: All patients under 18 years of age must be accompanied by a parent or legal guardianduring all office visits.
A pre-operative consultation and clinical examination is mandatory for patients undergoing IV anesthesia for surgery. At this appointment you will receive specific instructions regarding your planned surgical visit. You must refrain from all food and drink 6 hours prior to your surgery and we require that a responsible adult assist and drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.). These issues will be identified in our comprehensive Health and History form that you will complete during your initial visit (For your convenience a copy of the form is posted on this web site.)
